Important Notes:

All the Assessments and the accompanying Score Reports are free of charge.

When taking the assessment, use the "Back" button and "Next" button on the bottom of each page to move back and forth between pages.

Directions for Caregivers

To take an assessment:

  1. Click on the Assessments tab at the top of this page and select "Proceed to Assessments".
  2. Choose the caregiver assessment you want to take and click the "Begin" button. If you are unsure of which assessment to take, please follow the link provided that will help you select the appropriate assessment.
  3. Read the Terms and Conditions information page. After you are done, and if you agree, click on the "Yes, I Agree" button.
  4. Fill out the information the Login Information page. When you are done, click on the "Next" button.
  5. Read the brief instructions and click on the "Next" button to begin taking the assessment. Use the scroll bar on the right side of the pages to make sure you answer all the questions on each page. When you reach the end of the assessment, click on the "I'm Finished" button.
  6. Shortly after clicking on the "I'm Finished" button, the Score Report will "pop up" (it should take just a few moments). We strongly recommend printing the Score Report. It also can be saved on your computer.

The Score Report

Once you click on the "I'm Finished" button, the assessment is scored electronically and the Score Report will "pop up" in PDF format on your screen.

If on the Login Information page you entered an e-mail address(es) to receive a copy of the Score Report, the Score Report is e-mailed immediately after clicking on the "I'm Finished" button. The Score Report is sent in the e-mail as a PDF attachment. Simply click on that attachment to open the Score Report.

If you want both youth and caregiver(s) scores on the same report (a Matching Report), the youth and caregivers must use the same Youth ID and enter the same e-mail address in the "Matching E-mail Address" section on the Login Information page. The matching of scores will appear when two or more of the assessments have been completed, and if completed within 42 days of each other.

The Matching Score Report is e-mailed immediately after clicking on the "I'm Finished" button. The Score Report is sent in the e-mail as a PDF attachment. Simply click on that attachment to open the Score Report.

Directions for Youths

To take an assessment:

  1. Click on the Assessments tab at the top of this page and select "Proceed to Assessments".
  2. Choose the youth assessment you want to take and click the "Begin" button. If you are unsure of which assessment to take, please follow the link provided that will help you select the appropriate assessment.
  3. Read the Terms and Conditions information page. When you are done, and if you agree, click on the "Yes, I Agree" button.
  4. At the Login Information page, type the information that your caregiver (social worker, foster parent, teacher, etc.) gave you. If they didn't give you any information, ask them for it. When you are done, click on the "Next" button.
  5. Read the short instructions and click on the "Next" button to begin taking the assessment. Use the scroll bar on the right side of the pages to make sure you answer all the questions on each page.
  6. Answer the questions about yourself the best you can. When you reach the end, click on the "I'm Finished" button.
  7. Shortly after clicking on the "I'm Finished" button, the Score Report will "pop up" on your screen. Please print the Score Report so you talk about it with your parent, teacher or social worker. You can also save it on your computer.
